3-200-35: Import of Wild Collected Appendix-I Plants under CITES

This application should be used for the import of CITES-listed Appendix-I plants that have been collected from the wild. Please note that import of wild-collected specimens of Appendix-I plants cannot be for primarily commercial purposes, as outlined in CITES Resolution Conf. 5.10 (Rev. CoP15). For species that are protected by the U.S. Endangered Species Act, please use form 3-200-36 instead of this form since additional questions related to the Endangered Species Act must be addressed. 

To Complete This Form You Will Need the Following: 

  • Answer questions about the species you are planning to import. 
  • A copy of any required foreign permits to export the requested specimens or documents from the foreign government or other entities that indicate that the specimens were legally collected from the wild (e.g., collection permits, landowner permission). 
  • To attach photographs and a complete description of the facilities where plants will be held within the United States. If applicable, provide copies of your United States Department of Agriculture Protected Plant Permit and State permits related to this import of plants or plant products and subsequent propagation. 
  • Please note in addition to obtaining this permit for your proposed activity, for any commercial activity you will need to obtain an import / export license from our Office of Law Enforcement.
