Use this application to request a replacement document if your currently valid permit has been lost, destroyed, or misplaced (for this application, all permits, registrations, and certificates are referred to as a permit). Do not submit this application without identifying the permit to be replaced due to loss, misplacement, or destruction. If you need to replace a partially complete, single use certificate, ("clone"), you will need to download this application form and mail it to the office identified on the form.
To Complete This Form You Will Need the Following
- Identify the permit number (found in the upper right corner) of the permit needing replacement, if known.
- You may request replacement directly from your ePermits portal by selecting the permit you wish to replace and click the "replace/duplicate" button
- A variety of additional documentation may be needed depending on the circumstances. Please see the application form for more detail.
