3-200-37d: Interstate or Foreign Commerce of Live Animals/Samples/or Products (ESA)

This application covers activities involving interstate commerce in non-native ESA-listed animal specimens or foreign commerce in native and non-native ESA-listed animal specimens, whether live or dead, including any readily recognizable parts, products, or derivatives.  

To Complete This Form You Will Need the Following: 

  • Source of specimen (captive-bred or wild).  
  • Signed and dated breeder’s statement or other appropriate documentation depending on the source of the specimen (e.g. Species360 report for captive bred specimens or collection permits for wild source specimens). 
  • Documentation demonstrating that the proposed activity will enhance the propagation or survival of the species. 
  • Qualifications and resumes of the staff responsible for the proposed activity.  
  • Information on the receiving institution’s existing facilities, if applicable.  
  • Any documents to show the legal acquisition and transfer of the specimens.  
  • Please note in addition to obtaining this permit for your proposed activity, for any commercial activity you will need to obtain an import/export license from our Office of Law Enforcement.
