This application is for the export of LIVE CITES and/or ESA listed animals for the purpose of bona fide conservation breeding under a valid Captive Bred Wildlife (CBW) registration.
To Complete This Form You Will Need the Following:
- Copy of your valid CBW registration.
- Signed and dated breeder’s statement or other appropriate documentation (e.g. Species360 report).
- For ESA-listed species, documentation demonstrating that the proposed activity will be for bona fide conservation breeding.
- Qualifications and resumes of the staff responsible for the care and husbandry of the specimens at both the sending and receiving institutions.
- Information on the receiving institution’s existing facilities.
- For the export of CITES Appendix I specimens, a copy of the CITES import permit from the foreign CITES Management Authority, or confirmation that one will be issued.
- Please note in addition to obtaining this permit for your proposed activity, for any commercial activity you will need to obtain an import / export license from our Office of Law Enforcement.
