This application covers activities involving LIVE CITES Appendix-I and/or ESA listed animals.
To Complete This Form You Will Need the Following:
- Signed and dated breeder’s statement or other appropriate documentation depending on the source of the specimen (e.g. Species360 report for captive bred specimens or collection permits for wild source specimens).
- For ESA-listed species, documentation demonstrating that the proposed activity will enhance the propagation or survival of the species.
- Qualifications and resumes of the staff responsible for the care and husbandry of the specimens at both the sending and receiving institutions.
- Information on the receiving institution’s existing facilities.
- For the import of CITES Appendix I specimens, documentation showing that the activity will not be for primarily commercial purposes.
- For the export of CITES Appendix I specimens, a copy of the CITES import permit from the foreign CITES Management Authority, or confirmation that one will be issued.
- Please note in addition to obtaining this permit for your proposed activity, for any commercial activity you will need to obtain an import / export license from our Office of Law Enforcement.
