3-200-29: Export/Re-export of Wildlife Samples and/or Biomedical Samples (CITES)

This application is used to apply for export or re-export of biological samples for biomedical or scientific purposes for CITES- Appendix II or III listed species only. For U.S. Endangered Species Act listed species or CITES Appendix I species, you will need to use application form 3-200-37e.

Master File

Master Files may be obtained for overarching programs that require multiple exports. Once your program is approved, a Master File may be valid for at least one year, not to exceed 3. You would then apply for individual, partially complete permits which you would customize for each shipment. This form can also be used to renew or amend Master Files.

You must wait for a Master File to be established, renewed, or amended, prior to applying for individual, partially complete permits. You will be notified that your Master File is valid so you can apply for the permits associated with your Master File.

To Complete This Form You Will Need the Following:

  • Depending on your requested activity, you might be required to attach a variety of supporting documentation that demonstrates prior authorization to export, import, or re-export. 
  • A copy of any applicable local, Tribal, State, or Federal government authorization approving the activity.
  • Documentation that animals or samples were legally acquired. 
  • Please note in addition to obtaining this permit for your proposed activity, for any commercial activity you will need to obtain an import / export license from our Office of Law Enforcement.
