This site contains only public permit forms.

All Internal FWS forms are now located only on the FWS internal SharePoint site
(accessible to FWS employees with PIV card).

The Service has developed a new Permitting System - ePermits.

ePermits is a new system that will enable an applicant to easily submit FWS permit applications online while providing FWS users the ability to process more than 80 digital permit applications through a centralized, common platform. If you wish to continue to mail in your application, there is a hard copy form in ePermits that you can download, print, and mail in.

For Migratory Bird Program permits, to access a link to apply online via ePermits or to download a pdf version of the application or report form, please see the Migratory Bird Program's Need a Permit webpage. IMPORTANT: Before you select "Apply Online", please make sure to pay attention to the introductory information on this page about setting up or reclaiming an ePermits account.
