How to get the Adobe Acrobat 4.0 software on my computer?
 
These forms are available as separate PDF files.
 
When you select a form, the Adobe Acrobat Reader program will ask - "What do you want to do with this file?"
To fill-in and/or print this file only, you should "Open it". 
 To keep a copy of the blank form on your hard disk, 
                       you should  "Save it to disk".

PDF files must be downloaded and read using the most current Adobe Acrobat Reader 4.0.
This software is free. If you do not have the current version  installed on your computer, it is available from the Adobe Inc. site at http://www.adobe.com/prodindex/acrobat/readstep.html#reader.
(BEFORE INSTALLING ADOBE ACROBAT READER 4.0, YOU MUST UNINSTALL ALL PREVIOUS VERSIONS AND RESTART YOUR COMPUTER FIRST). 

Installation Instructions  

First, click on the above link to get to the Adobe site. 
The default setting will provide the installation program for the Windows version of the software.  Click on the "Download" link. 

You will be asked to provide a directory location on your computer that will hold the installation program (ar40eng.exe).  Remember the directory location. 

 After the download is complete:  

1. Go to the directory where the newly downloaded file was put. (Using Microsoft Explorer) 
2. Double-click the newly downloaded file (ar40eng.exe). 
3. Follow the instructions on your screen. 

If there is a failure at any point during the installation of Acrobat Reader, the installer performs a complete uninstall. For this reason, it is important not to close the installer application by clicking its close box in the upper right corner of the background window after clicking the "Thank You" dialog box that appears at the end of the installation. If you wait for a second or two, the installer will automatically close the background windows after the installation is complete.
